What is the CVLT-3? Inside the California Verbal Learning Test
The CVLT-3, or California Verbal Learning Test, Third Edition, is a clinician-administered memory test in which a person tries to learn a list of 16 spoken words over five repeated readings, then recalls those words after distraction and after a 20-minute delay. Published by Pearson in 2017 and authored by Dean C. Delis, Joel H. Kramer, Edith Kaplan, and Beth A. Ober, it is designed for people ages 16 through 90. Because it tracks how learning unfolds trial by trial, the CVLT-3 can help a neuropsychologist tell whether a memory complaint reflects trouble getting information in, holding onto it, or pulling it back out. This article explains how the test is built, what its scores mean, the children's and brief versions, who uses it, and how it relates to IQ testing.


What the CVLT-3 measures

The CVLT-3 is a test of "verbal learning," the ability to acquire new spoken information through repetition, and "episodic memory," the ability to retain and retrieve that information later. According to Pearson, it is the most comprehensive assessment of verbal learning and memory available for older adolescents and adults, and the standard form takes about 30 minutes to administer plus a 30-minute delay period before the final trials.

An earlier edition of the test was described by researchers writing in the Archives of Clinical Neuropsychology as one of the most widely used neuropsychological tests in North America, and the third edition carries that tradition forward with a full restandardization. The publisher reports that the new norms account for education, region, ethnicity, and age.

The word lists are deliberately structured. Each list contains 16 words drawn from four everyday semantic categories, four words per category, and the words were selected for their frequency of use across demographic groups. That structure matters because it lets examiners see whether a person notices and uses the categories to organize learning, a point covered in more detail below.


How the test works, step by step

The CVLT-3 follows a fixed sequence that Pearson documents in its scoring materials. A typical administration looks like this.

• Learning trials 1 through 5: The examiner reads List A, 16 words, aloud. The examinee repeats back as many words as possible, in any order. This happens five times with the same list, which produces a "learning curve" showing how much new information is gained with each repetition.

• Interference trial: A second 16-word list, List B, is read once and recalled once. This mimics everyday interference, the way new information can push aside what you just learned.

• Short-delay recall: The examinee is asked for List A again, first freely and then with category cues (for example, being reminded that some words were types of vegetables). Comparing "free recall" with "cued recall" shows whether cues help the person find memories they could not produce on their own.

• Long-delay recall and recognition: After about 20 minutes of unrelated activity, the examinee recalls List A once more, freely and with cues, and then completes a yes/no "recognition" trial, deciding whether each word presented was on the original list. An optional forced-choice recognition trial can follow after a further 10-minute delay, which also helps in evaluating insufficient effort.

The full battery yields core scores plus expanded scores for deeper analysis of errors, including "intrusions" (words recalled that were never on the list) and repetitions.


What the learning curve and process scores reveal

The value of the CVLT-3 lies less in any single number than in the pattern across trials. Memory researchers commonly divide remembering into three stages: "encoding" (getting information into memory), "storage" (keeping it there), and "retrieval" (getting it back out). The test's design allows each stage to be examined somewhat separately, which is one reason list-learning tasks feature so heavily in memory assessment. A flat learning curve across the five trials suggests an encoding problem: repetition is not producing gains. Strong learning followed by a sharp drop after the 20-minute delay points instead toward a storage or "consolidation" problem, since the information was acquired but did not survive the wait. A third pattern, poor free recall alongside intact recognition, suggests a retrieval problem: the memories exist, because the person can pick them out when shown the words, yet cannot summon them unaided.

Strategy scores add another layer. Because the list is built from four categories, examiners can compute a chance-adjusted "semantic clustering" score reflecting how often a person recalls same-category words together. Research published in Brain Sciences in 2024 found that people who spontaneously grouped words by meaning learned more words by the fifth trial and recalled far more after both short and long delays, and that exceptionally sharp older adults, sometimes called superagers, used the strategy more than typical peers.

Error analysis can even help distinguish conditions. A 2019 study in the Journal of the International Neuropsychological Society, co-authored by test author Dean Delis, reported that people with Alzheimer's disease tended to produce novel intrusion errors, consistent with faulty encoding and storage tied to medial temporal brain regions, while people with Huntington's disease produced more repeated intrusions, consistent with retrieval and executive difficulties tied to subcortical-frontal circuits.


Versions: the CVLT-C, the Brief form, and the alternate form

The CVLT family covers different ages and time budgets.

• CVLT-C (Children's Version): Published in 1994 for ages 5 through 16 years 11 months, the CVLT-C applies the same list-learning approach to children. Pearson reports administration of about 15 to 20 minutes plus a 20-minute delay interval, with 27 indices covering recall, learning characteristics, and error types. It is used with children who have traumatic brain injuries, learning disabilities, and attention difficulties.

• CVLT-3 Brief form: For patients who cannot tolerate a full session, the Brief form uses lists of nine words in three categories and takes about 15 minutes, per the publisher.

• Alternate form: The CVLT-3 also offers an equivalent alternate word list. This matters because of "practice effects," score gains that come from having taken the test before rather than from real memory change. A study of 195 healthy adults retested after one month found that people who took the same form twice improved by roughly a quarter to three fifths of a standard deviation on the main indices, while those given the alternate form at retest showed essentially no practice gain. Reliability stayed reasonably high either way, which is why longitudinal evaluations often rotate forms.


Who uses the CVLT-3, and for what

Common settings include dementia evaluations, where the delayed-recall and error profiles help separate Alzheimer's-type memory loss from other conditions; traumatic brain injury assessments, where a 2002 study of 160 patients found CVLT profiles sorted into a normal-range group and a moderately-to-severely impaired group; and epilepsy programs, where verbal memory testing informs surgical planning. Honesty about limits is warranted here: a 2009 study of 61 epilepsy patients found that while the CVLT and similar tests distinguished mesial temporal sclerosis from other pathologies, their ability to lateralize a seizure focus to the left or right hemisphere was poor, and different verbal memory tests are not interchangeable.

The forced-choice recognition trial also gives examiners a built-in check on effort, since even people with severe memory disorders perform well above chance when simply choosing between two options.


How the CVLT-3 differs from an IQ test

The CVLT-3 is a memory instrument, and it measures something most IQ tests barely touch: retention of new material over delays of 20 minutes or more. Tests like the WAIS assess "working memory," the ability to hold and manipulate information over seconds, but they do not track whether learned material survives half an hour. The two abilities are related yet distinct, and a person can score well on one while struggling on the other. That is why a thorough evaluation of a memory complaint pairs an intelligence test with a dedicated memory measure.
