The framework then considers "self-regulation," or what the person does about that input. Passive self-regulation means letting sensory experiences happen and reacting afterward; active self-regulation means taking steps to manage or control the input. The four sensory patterns
The quadrant scores are the signature feature of the instrument. Research on the Sensory Profile 2 labels the four scored patterns Seeking, Avoiding, Sensitivity, and Registration, and Pearson describes the children who show each pattern strongly in the following terms.
• Seeking (high threshold, active regulation): children with elevated seeking scores are described as busier and more engaged with sensory experiences than peers. They pursue extra input, such as touching objects, moving constantly, or making noise.
• Avoiding (low threshold, active regulation): children with elevated avoiding scores are more likely to retreat from unfamiliar or intense situations. They work to limit the sensory input coming at them.
• Sensitivity (low threshold, passive regulation): children Pearson calls "Sensors" react more quickly and more intensely to stimuli than others. They notice input readily but do not act in advance to control it.
• Registration (high threshold, passive regulation): children Pearson calls "Bystanders" miss more sensory cues than their peers. They may not notice their name being called or food on their face.
Higher or lower scores in a quadrant are compared with norms, and the publisher stresses that a pattern only matters clinically when it interferes with participation in everyday life. Occupational therapists use the pattern profile to plan supports, such as adding sensory value to tasks for seekers or reducing sensory load for avoiders.
Forms, age ranges, and languages

How the Sensory Profile 2 is administered and scored
Pearson reports a completion time of 5 to 20 minutes per form, with paper-and-pencil or online administration. Raters mark how frequently the child shows each behavior, and forms are scored either by hand or through Q-global, Pearson's web-based scoring platform. Depending on the form, results include Sensory System scores (for example, auditory or tactile processing), Behavioral scores, Sensory Pattern (quadrant) scores, and School Factor scores on the teacher form. The system uses cut scores with optional percentiles, so a child's ratings are interpreted relative to a normative sample rather than by raw counts alone.

Strengths, limitations, and how it differs from an IQ test
A rating scale and an intelligence test answer different questions with different methods. An IQ test is a performance measure: the examinee solves problems, and the score reflects demonstrated reasoning under standard conditions. The Sensory Profile 2 instead quantifies an observer's impressions of behavior. That design is efficient and captures real-world functioning across settings, but it also means scores depend on the rater. Two adults can rate the same child differently because they see the child in different contexts or hold different expectations, which is why the multi-rater comparison exists. Rater effects are a general property of questionnaire measures, including behavior rating scales such as the BASC-3 and the SRS-2, and they are one reason schools that rely heavily on ratings can misjudge a child's underlying abilities, a concern discussed in the video above.
The evidence base for the Sensory Profile 2 has genuine strengths and documented gaps. A 2023 review by Licciardi and Brown in the Scandinavian Journal of Occupational Therapy, using the COSMIN standards for health measurement instruments, concluded that the instrument shows strengths in its design and "internal consistency," the extent to which items on a scale hang together. The same review found notable shortcomings in structural validity, cross-cultural validity, and hypothesis testing, and reported that the manual offered insufficient evidence on responsiveness and criterion validity. The authors judged the psychometric properties adequate overall while urging users to keep the shortcomings in mind. Translation studies continue to extend the tool internationally; a 2026 study in the journal Children, for instance, reported good reliability for a Greek adaptation of the short form.
Two boundaries matter for parents reading a Sensory Profile 2 report. First, it does not measure intelligence, academic skill, or perceptual accuracy; a performance measure such as the MVPT-4 assesses visual perception by having the child actually do tasks, which is a fundamentally different kind of evidence. Second, it does not diagnose any condition. Elevated scores describe sensory patterns, and only a qualified clinician can determine what those patterns mean within a full evaluation.
Where cognitive testing fits in
The Sensory Profile 2 tells a team how a child experiences sensory input; it says nothing about reasoning ability. When the question is cognitive, a measure built for that purpose is required. For adults 18 and older, the Reasoning and Intelligence Online Test, developed by RIOT IQ with psychometrician Dr. Russell T. Warne, measures cognitive ability with 15 subtests across six indices: verbal reasoning, fluid reasoning, spatial ability, working memory, processing speed, and reaction time.
