The Wisconsin Card Sorting Test, usually abbreviated WCST, is a neuropsychological test of "set-shifting," the ability to abandon a strategy that has stopped working and adopt a new one. The examinee sorts cards that vary in color, shape, and number, guided only by feedback about whether each sort was right or wrong. Without warning, the sorting rule changes, and the test records how quickly the person notices and adapts. Where the Wisconsin Card Sorting Test came from
Card sorting has a long history in psychology. A review by Erika Nyhus and Francisco Barceló in the journal Brain and Cognition traces the tradition back to sorting tasks used in the early 1900s, and notes that the WCST itself was devised in 1948 by David Grant and Esta Berg as an index of abstract reasoning, concept formation, and the ability to respond to changing contingencies. Berg introduced the technique in The Journal of General Psychology as "a simple objective technique for measuring flexibility in thinking," and Grant and Berg published a companion study of rule shifting in the Journal of Experimental Psychology the same year. Both authors worked at the University of Wisconsin, which gave the test its name.
The test's clinical reputation was built in the following decades. According to the Nyhus and Barceló review, the neuropsychologist Brenda Milner introduced the WCST as a tool for assessing prefrontal dysfunction in patients with brain lesions in 1963, reporting that damage to the frontal lobes was associated with difficulty shifting sorting rules. Robert Heaton and colleagues later standardized the administration and scoring procedures, and their professional manual, revised in 1993, remains the basis of the version used today. The test is currently published by PAR, Inc., which lists Grant and Berg as authors and Heaton, Chelune, Talley, Kay, and Curtiss as authors of the professional manual.
How the sorting task works
The materials are deliberately simple. In the conventional format described by Nyhus and Barceló, four "stimulus cards" showing geometric figures are placed in a row in front of the examinee, and the examinee receives a deck of response cards (128 in the full version) whose figures vary along three dimensions: color, form, and number. The task is to place each response card under one of the four stimulus cards. The examiner never explains the sorting rule. After each placement, the examinee is told only whether the match was correct.
Through trial and error, most people discover the active rule, for example matching by color. The critical moment comes after ten consecutive correct sorts, when the rule silently changes. Sorts that were right a moment ago are suddenly wrong, and the examinee must notice the change, let go of the old rule, and search for the new one. The test is untimed and continues until the examinee has completed six rule categories or run out of cards. According to the publisher, a standard administration takes about 20 to 30 minutes, and normative data cover ages 6 and a half through 89.

Key scores and what perseveration means
The standardized scoring system generates many indices, but in practice a few carry most of the interpretive weight. Nyhus and Barceló note that although the Heaton norms offer sixteen different scores, most researchers and clinicians rely on two or three.
• Categories completed: the number of sorting rules (up to six) the examinee successfully worked out and maintained for ten consecutive correct sorts. Higher is better.
• Perseverative responses and perseverative errors: "perseveration" means continuing to sort by a rule after feedback shows it no longer applies. A perseverative error is an incorrect sort that sticks with the previously reinforced principle. Elevated perseverative errors are the test's signature finding and suggest difficulty disengaging from an outdated strategy.
• Non-perseverative errors: incorrect sorts that do not follow the old rule. These often reflect lapses of attention, guessing, or losing track of the current rule rather than rigidity.

The distinction matters because two people can make the same number of total errors for different reasons. A profile dominated by perseverative errors points toward inflexibility, while scattered non-perseverative errors raise questions about sustained attention or working memory.
The WCST-64 and computerized versions
Because the full 128-card test can run long, a short form exists. According to PAR, the WCST-64 (authored by Kongs, Thompson, Iverson, and Heaton) uses only the first 64 cards of the standard deck, cutting administration to roughly 10 to 15 minutes while keeping the task requirements intact. The publisher notes that the fixed 64-card length also removes variability in how many cards are administered, which makes retest comparisons more straightforward. Its demographically corrected norms come from the same samples as the full test, comprising 452 children and adolescents and 445 adults.
Administration has also moved onscreen. Who takes the WCST, and what it can and cannot show
The WCST typically appears inside a broader evaluation rather than on its own. The test also has a long research career in psychiatry. In a study published in the Indian Journal of Psychiatry, Singh and colleagues reported that patients with schizophrenia performed worse than healthy comparison participants on essentially every WCST index except the total number of correct responses, and that patients with predominantly negative symptoms showed the most pronounced perseverative difficulties. Findings like these have made the WCST a standard tool for studying cognitive impairment in schizophrenia.
Honest limitations deserve equal billing. The same Nyhus and Barceló review that documents the test's history argues that the WCST, in its traditional form, fails to discriminate between frontal and non-frontal brain lesions, and that imaging studies show rapid, widespread activation across frontal and non-frontal regions during the task. In other words, a poor WCST score flags an executive problem worth investigating, and it should never be read on its own as proof of frontal-lobe damage.
