The Bayley-4, formally the Bayley Scales of Infant and Toddler Development, Fourth Edition, is an individually administered developmental assessment for children aged 16 days to 42 months, published by Pearson in 2019. A trained examiner uses play-based tasks to measure cognitive, language, and motor development directly, while caregivers complete questionnaires covering social-emotional and adaptive behavior. Clinicians use the results to identify developmental delays and qualify children for early intervention services. This article explains what the Bayley-4 measures, how it is administered and scored, what its scores can and cannot tell you about a child's future, and how it differs from the IQ tests adults take.
What the Bayley-4 measures
According to Pearson, the Bayley-4 assesses development across five domains. Three are measured through direct assessment of the child, and two through structured caregiver questionnaires.
• Cognitive: how the child explores, attends, solves simple problems, and manipulates objects. At young ages this looks like tracking a ring or searching for a hidden toy; at older ages it involves puzzles and pretend play.
• Language: split into receptive communication (understanding words, following simple directions) and expressive communication (babbling, naming objects, combining words).
• Motor: split into fine motor skills (grasping, stacking blocks, scribbling) and gross motor skills (sitting, crawling, walking, climbing stairs).
• Social-emotional: reported by a caregiver, covering how the child responds to people, communicates needs, and uses emotional signals. Pearson's sample score report shows this scale asking, for example, how the child responds to sounds, touch, and other things in the environment.
• Adaptive behavior: also caregiver-reported, covering everyday independence in areas such as communication, socialization, and daily living skills.
The direct scales and the questionnaire scales serve different purposes. The examiner-administered tasks capture what the child can do under standardized conditions on one day, while the caregiver questionnaires capture typical behavior at home over time. A thorough evaluation uses both views together.
How the Bayley-4 is administered
The Bayley-4 is administered one-on-one by a trained examiner, and the publisher restricts purchase to professionals with relevant graduate training (Pearson's qualification Level B). Sessions take about 30 to 70 minutes depending on the child's age, and the test can be given in paper format or digitally through Pearson's Q-global platform.
Administration is deliberately play-based. Because an infant cannot follow verbal test instructions, the examiner presents toys, blocks, pictures, and simple games in a set sequence and observes how the child responds. Items are scored against detailed criteria, and the child's performance is compared with a normative sample that Pearson reports was aligned with recent US census data and included children from culturally and linguistically diverse backgrounds.
Two administration details matter for interpretation. First, the examiner starts at an age-based item set, so a 9-month-old and a 30-month-old see very different tasks. Second, for children born prematurely (36 weeks gestation or earlier), Pearson recommends using "adjusted age," which subtracts the weeks of prematurity from chronological age, for up to 24 months. A baby born two months early is compared with the norms for a younger age group, which prevents ordinary prematurity from being misread as delay.
How Bayley-4 scores work
Pearson reports that the Bayley-4 yields subtest-level "scaled scores" (mean of 10, standard deviation of 3), domain-level "composite scores" on the familiar standard-score metric (mean of 100, standard deviation of 15), plus percentile ranks, confidence intervals, developmental age equivalents, and growth scale values. A composite of 100 means performance at the median for the child's exact age group, and the publisher's sample reports display these scores with confidence intervals rather than as single fixed numbers.
You will sometimes hear infant test results described as a "developmental quotient," or DQ. Historically, a DQ was a ratio: the child's developmental age divided by chronological age, multiplied by 100, much like the original ratio IQ built on mental age. Modern instruments such as the Bayley-4 use deviation-based standard scores instead, though clinicians still use "DQ" loosely to mean any developmental composite.
What a developmental quotient does mean: it locates a child's current skills relative to same-age peers, flags domains that lag meaningfully behind the norm, and provides a defensible number for eligibility decisions and for tracking change after intervention.
What it does not mean: it is a snapshot of infancy, and it is neither an IQ score nor a forecast of one. A developmental composite of 85 at 18 months describes skills at 18 months. It does not predict what the same child's IQ will be at age 8, and it says nothing about eventual adult ability. The next section explains why.
Why infant scores are weak predictors of later IQ
This is one of the best-documented findings in developmental psychometrics: scores on infant developmental scales correlate weakly with IQ measured years later, especially for children without major medical complications.
The evidence is direct. In a 2005 study in Pediatrics, Maureen Hack and colleagues followed 200 extremely low birth weight children who took the Bayley scales (the second edition) at 20 months and a standardized cognitive battery at age 8. Among children who scored below 70 as toddlers, only 37 percent still scored below 70 at school age, and the overall rate of scores in the impaired range fell from 39 percent to 16 percent. The authors concluded that the predictive validity of a subnormal infant score for school-age cognitive function is poor, even in this high-risk group where prediction tends to be strongest.
The broader literature agrees. A meta-analysis by Robert McCall and Michael Carriger in Child Development examined how well infant measures predict childhood IQ and found that standardized infant developmental tests were consistently outperformed by simple laboratory measures of infant attention, such as habituation and recognition memory, which themselves reached only moderate correlations (a weighted average around .36) with IQ between ages 1 and 8.
There is a sensible reason for this. Infant scales necessarily measure sensorimotor milestones, things like grasping, babbling, and object search, because those are the behaviors infants can produce. Later IQ tests measure reasoning, vocabulary, and abstract problem solving. These are different constructs, and early motor precocity has little bearing on later abstract reasoning. Development is also genuinely uneven: children accelerate and plateau on their own schedules, which is the same reason age-based expectations mislead in classrooms, as Karen Rambo-Hernandez discusses in the video above.
None of this makes the Bayley-4 a weak test. It is well suited to its actual job, which is describing current development and identifying children who need help now. The error lies in reading it as a crystal ball.
Who uses the Bayley-4
• Early intervention programs: Under Part C of the Individuals with Disabilities Education Act, a federal grant program described by the ECTA Center, states run early intervention services for infants and toddlers with disabilities from birth through age 2. Standardized developmental scores such as the Bayley-4's are commonly part of the evaluations that determine eligibility.
• NICU and high-risk follow-up: A 2021 clinical review in Archives of Disease in Childhood describes the Bayley scales as a gold-standard assessment routinely used to monitor children at risk of delay, including those born preterm, because early identification allows intervention when it is most effective.
• Research: the same review notes the Bayley is a common endpoint in neonatal clinical trials, where developmental composites serve as outcome measures for medical interventions.
Once a child ages out at 42 months, examiners typically move to preschool intelligence tests such as the WPPSI, which introduce IQ-style reasoning tasks and produce scores that become progressively more stable with age.
Where adult IQ testing fits
The Bayley-4 and adult IQ tests sit at opposite ends of the lifespan and measure different things. The Bayley-4 has no adult equivalent and no online substitute.
