These two terms are used interchangeably in everyday conversation, and they shouldn't be. Taking an IQ test is relevant to both conditions, but in opposite directions, and the diagnostic criteria, support needs, life outcomes, and appropriate interventions differ substantially between them. Getting this distinction right matters practically for parents, educators, clinicians, and anyone trying to make sense of a diagnosis. This article explains what each condition actually is, what the IQ profile looks like in each case, where the confusion comes from, and what the research says about appropriate support.
The Fundamental Difference in One Sentence
An intellectual disability affects general cognitive functioning across multiple domains of life. A learning disability affects a specific cognitive process, most commonly reading, writing, or mathematics, while leaving general intelligence intact.
That distinction has a direct implication for IQ scores. Intellectual disability is characterized by a below-average IQ level (IQ below 70), while learning disability is characterized by an average or above-average IQ level (IQ above 85). The IQ criterion is not the whole story for either condition but it captures the most fundamental difference: one condition involves broadly reduced cognitive ability, and the other involves a specific deficit that coexists with otherwise normal or above-normal intelligence.
What Intellectual Disability Actually Is
An intellectual disability (ID) involves significant limitations in both intellectual functioning and adaptive behavior, originating before age 18. Under the DSM-5 and the framework of the American Association on Intellectual and Developmental Disabilities (AAIDD), both components must be present for a diagnosis.
Intellectual functioning refers to the general mental abilities measured by a standardized IQ test: reasoning, problem-solving, planning, abstract thinking, and learning from experience. An IQ score below approximately 70, which is two standard deviations below the population mean, is the psychometric threshold. But the DSM-5 explicitly emphasizes that the IQ score alone is insufficient for diagnosis; it must be accompanied by clinically significant deficits in adaptive behavior.
Adaptive behavior refers to the practical, everyday skills required for functioning in daily life: communication, self-care, social interaction, home living, health and safety, academic skills, and use of community resources. An intellectual disability involves problems with general mental abilities that affect functioning in two key areas: intellectual functioning and adaptive functioning. A person with ID has an IQ below 70, and they find daily living activities, social skills, and conceptual and practical skills difficult; impairments that develop before adulthood and affect multiple life domains, not just academic performance.
Intellectual disability affects approximately 1% of the population, and an estimated 85% of those affected have what is classified as mild intellectual disability. The severity classification (mild, moderate, severe, and profound) reflects the degree of support needed rather than a fixed numerical IQ boundary. A person with mild ID (IQ typically 50–70) may live semi-independently, maintain employment in supported settings, and develop many practical skills. A person with profound ID requires continuous support for all aspects of daily living.
The causes of intellectual disability span a wide range. Intellectual disability can be caused by genetic conditions, issues during pregnancy, birth complications, or environmental factors such as severe early neglect or lead poisoning. Down syndrome, Fragile X syndrome, and fetal alcohol spectrum disorder are among the most recognized causes.
What a Learning Disability Actually Is
A learning disability (LD) is a neurologically-based processing disorder that interferes with the acquisition and use of specific academic skills, most commonly reading, written expression, or mathematics, in a person of otherwise average or above-average intelligence.
The most important conceptual feature of a learning disability is the discrepancy between overall cognitive ability and specific academic performance. People with learning disabilities have specific issues with certain types of learning but have average or above-average IQ. The learning challenges cannot be caused by low intelligence; that exclusion criterion is built into the diagnostic framework. If a child's reading difficulties are fully explained by a general intellectual disability, the specific diagnosis of dyslexia is not applied. The most common learning disabilities are:
Dyslexia — difficulty with accurate and fluent word reading and spelling, caused by a deficit in the phonological processing system. Dyslexia is the most prevalent learning disability, affecting an estimated 5–15% of the population, and it is independent of intelligence. Dyscalculia — difficulty with number sense, arithmetic fact retrieval, and mathematical reasoning, in the presence of otherwise normal cognitive functioning. Dyscalculia affects an estimated 3–7% of children and is distinct from the mathematical difficulty that would be expected given a general intellectual disability. Dysgraphia — difficulty with written expression, including handwriting, spelling, and organizing written language, not explained by motor impairment or general cognitive limitation.
Nonverbal learning disability (NVLD) — a pattern of strong verbal ability alongside significant weaknesses in visual-spatial processing, mathematics, and social comprehension; a profile that produces distinctive academic and social challenges despite intact language and verbal reasoning.
The neurological basis of learning disabilities is specific and well-documented. Learning disabilities involve differences in how the brain processes information in the domains affected; phonological processing in dyslexia, numerical magnitude processing in dyscalculia, visuomotor integration in dysgraphia. These processing differences are present in the context of a brain that handles most other cognitive tasks normally.
The UK Terminology Problem: A Source of Major Confusion
One major source of confusion about these terms is geographic, and understanding it is essential for anyone reading research or clinical literature from multiple countries.
In the UK, they call learning disabilities "learning difficulties," and they call intellectual disabilities "learning disabilities". This is the opposite of American usage. A British clinical report describing a patient with a "learning disability" is describing someone with an intellectual disability in American terminology. An American report describing someone with a "learning disability" is describing a specific processing disorder in a person of normal intelligence — which would be called a "learning difficulty" in the UK.
This terminological inversion has caused genuine confusion in both popular and research contexts, and it affects how information is interpreted across national borders. When reading any source on these conditions, the first question to ask is which country's terminology the author is using. The answer changes the meaning of almost everything else in the document.
How IQ Assessment Relates to Each Condition
The role of IQ testing is meaningful but differently weighted in each diagnostic context.
For intellectual disability, an IQ score below approximately 70 is a necessary but not sufficient condition for diagnosis. A diagnosis requires both a low IQ score and documented deficits in adaptive behavior. A person with an IQ of 68 who functions independently, maintains employment, and navigates daily life without meaningful support would not meet the full diagnostic criteria; the adaptive component matters as much as the cognitive score. The DSM-5's shift toward emphasizing adaptive functioning over IQ cutoffs alone reflects the research literature's finding that adaptive competence is the more practically meaningful variable. For learning disabilities, IQ assessment serves a different function: it establishes that the specific academic deficit is genuinely discrepant from the person's general cognitive ability. The learning challenges cannot be caused by low intelligence — meaning that for a reading difficulty to be classified as dyslexia rather than as expected difficulty for someone with below-average IQ, the person's general intelligence must be in the average range or above. This is why IQ testing is a standard component of learning disability evaluations, not because LD involves any particular IQ score, but because the diagnosis requires ruling out general cognitive limitation as the explanation. The characteristic IQ profiles on Wechsler batteries differ between the two conditions in ways I've described in the neurodivergent profiles article in this series. An intellectual disability produces broadly depressed indices across all domains. A specific learning disability produces an uneven profile, typically with some indices preserved and others significantly depressed, that creates the inter-index scatter that signals a specific processing deficit rather than generalized cognitive limitation.
Age of Identification and Developmental Trajectory
The two conditions also differ in when they typically become apparent and how they unfold developmentally.
Intellectual disabilities are often recognized earlier, sometimes before a child turns three, because developmental milestones such as speech, movement, and problem-solving may progress more slowly. The delays are broad enough to be visible before formal academic demands begin. Early intervention programs (speech therapy, occupational therapy, developmental support) are typically implemented in the preschool years.
Learning disabilities, by contrast, generally become apparent once a child begins formal schooling and encounters structured reading and mathematics instruction. A child with dyslexia may develop normally in all observable ways until they are expected to decode print, at which point the specific phonological processing deficit becomes evident in a way that couldn't be detected before the developmental demand for that skill emerged. The late identification is a consequence of the domain specificity: a child functioning normally in most areas won't trigger developmental concern until the specific domain of weakness is formally assessed.
The long-term trajectory also differs. Many individuals with specific learning disabilities, with appropriate intervention and accommodation, go on to achieve at high academic and professional levels with specialized instruction, many go on to thrive academically and build successful careers. The processing difference doesn't disappear, but compensatory strategies and formal accommodations can substantially reduce its impact on outcomes. Intellectual disability has a more persistent effect on the full range of life domains (academic, occupational, social, and practical) and the support needs, while they may evolve, are lifelong.
Why the Distinction Matters Practically
Getting this distinction right is not a semantic exercise. It determines what kind of support a person receives, what educational programming is appropriate, what legal protections apply, and what outcomes are realistically achievable.
A child with dyslexia placed in a program designed for intellectual disability will be underchallenged, stigmatized, and denied the specific phonological intervention they actually need. A child with an intellectual disability identified as having only a learning disability will be placed in a general education setting without the adaptive support and modified curriculum that their functional needs require. Both errors happen and both happen because the distinction between the two conditions is genuinely poorly understood at the level of the general public, and sometimes even within educational systems.
The corrective is straightforward in principle: comprehensive cognitive and adaptive behavior assessment, conducted by qualified evaluators, that produces a full profile rather than a single label. A profile-based diagnosis identifies which specific domains are affected, at what severity, and with what functional implications, rather than assigning a category and hoping the category matches the person's actual needs.

Intellectual disability and learning disability are distinct conditions that affect cognition in fundamentally different ways. Intellectual disability involves broadly reduced general cognitive functioning alongside deficits in adaptive behavior, with IQ typically below 70 and support needs spanning multiple life domains. The IQ profile is one of the clearest diagnostic signals distinguishing the two — broadly depressed across all indices in intellectual disability, characteristically uneven with specific index depression in learning disability, and understanding that profile distinction is one of the most practically useful things a comprehensive cognitive assessment can provide.
